I fail to see how you can 'fall into a chipper'. I understand that sometimes people get caught up and dragged in. I actually witnessed a rope attached to a climber very nearly being thrown into a chipper with a load of brash, but 'falling in' is not easily done. The article linked to, mentioned that safe working practices were in use; I wouldn't agree.
This story is horrific, it just goes to show you can never take working with machinery for granted. Terrible for his family.

In America it's common for Tree Companies to use Whole Tree Chippers for everything which are easily large enough to take a person inside of them. I believe this is the same story I read on another forum where the article linked then mentioned that the guy was stood on top of the chipper cutting the branches so if he was knocked over and fell onto the hopper whilst the chipper was being loaded he could easily have been dragged in before anyone can react.
I doubt this was the type of chipper we have here!!!
