Wow phil you have been busy. Recently had to do one of these with guidance from the HSE. Client accepted it as extremely comprehensive but their H&S officer wasn't happy - never found out why as he broke his arm and never got back.(there's an irony in there somewhere!) We have another job with the same client so I reckon I'll use your format this time.

I produced these doc. in 2000 for my company. I decided at the time that I would cover all eventualities just in case.
